Chronic Disease Self-Management (CDSMP)
NARBHA has made a commitment to the integration of Stanford's Chronic Disease Self- Management Program (CDSMP) into the whole health services framework, incorporating both behavioral health services and physical health services as key components of treatment.
Through CDSMP training, patients can improve their overall health and health care experience through a proven, evidence-based self-management class. The class employs a new workforce of specially trained people with mental health/ substance use disorders (MH/SUD) as "peer recovery specialists."
By implementing widespread use of CDSMP, high risk behavioral health recipients will improve health, decrease hospitalizations and days in the hospital as validated in randomized control trials.
